SPARK HILLS, LLC - Lesson Agreement LESSONS ARE WEEKLY AND ONGOING - I wish to be scheduled for weekly music lessons. I agree to pay tuition monthly. There is a one month minimum enrollment period. TUITION IS FOR ENROLLMENT NOT ATTENDANCE OF LESSONS ??? If you miss a lesson you must still pay tuition but there are opportunities for makeup lessons (see below). MOST MONTHS HAVE 4 LESSONS - A few times per year there will be 5 lessons and you will be charged for 5. Due to holidays in December, some students will only receive 3 lessons and will only be charged for 3.

Please call or email customer service for cancellations and schedule changes! Your teacher cannot accept cancellations or withdrawals.

Missed Lessons CANCELLATIONS - If you cancel your lesson with LESS than 24 hours notice, the lesson cannot be made up and you will be charged for the lesson. Group classes cannot be cancelled, rescheduled or credited for any reason. MAKEUP LESSONS - If you cancel your lesson 24 hours or more in advance, you can request a makeup lesson. Makeups are subject to teacher availability and are NOT guaranteed. You can schedule makeup lessons up to three weeks in advance. NO REFUNDS for unused makeup lessons (even if you cannot find a time that you like). You can use makeups with any teacher at any 4/4 School location as long as you are enrolled. Makeup opportunities expire when you withdraw from the school and cannot be used to reduce tuition or extend enrollment.

You must submit a withdrawal form or call customer service in order to stop charges to your account.

Withdrawal and Refunds HOW TO WITHDRAW - We will continue to charge monthly tuition until you notify our OFFICE that you are withdrawing. Your teacher CANNOT withdraw you. You must notify our OFFICE no later than the 20th of the month. Your lessons will continue until the end of that month. To withdraw, simply call our office or go to ../withdraw_form. REFUNDS - No refunds unless your teacher cannot attend lessons. We reserve the right to terminate lessons for any student without notice. In this situation a refund for unused lessons will be given.

Photo ReleaseThe school is hereby granted permission to take photographs of the students to use in brochures, web sites, posters, advertisements and other promotional materials the school creates. Permission is also hereby granted for the school to copyright such photographs in its name.

OtherThe school reserves the right to provide a substitute teacher if the regularly scheduled teacher is ill or otherwise unable to attend a lesson. If the teacher is ill and the school cannot arrange a substitute, any missed lessons will be rescheduled. The school is not responsible for providing before or after class care for students nor is the school responsible for visitors at any time. Parents are asked to remain in the school during classes. Students are not to be left at the school before or after lessons. Parents, legal guardians, students & visitors waive the right to any legal action for any injury sustained at any school location or property resulting from the normal lesson activity or any other activity before, during, or after lesson time.
